How To Apply:
Step:1
Go to this link https://surokkha.gov.bd/
Step:2
Click the 1st Option of this website.
Step:3
Then select "ধরণ নির্বাচন করুন" & select your a catagory from this list which matches you.
Step:4
After that give your NID Card Number & give your Date Of Birth according to NID. Besides fill the captcha code. And Click On "যাচাই করুন" option.
Step:5
Then next page give your accurate Mobile Number and tap the all options of many kinds of diseases as Yes or, No option.
Also select your profession (পেশা) & your present address (বর্তমান ঠিকানা) where you are taking vaccine. After that click the button "সংরক্ষণ করুন".
Step:6
In your given Mobile Number, you get a OTP code within 5 Minutes. Then submit the OTP code here blank box. If the OTP code does not get in 5 Minutes then you can resend the OTP code here.
Step: 7
Then Click the button of "টিকা কার্ড সংগ্রহ".
And here also give your NID Card Number & Date Of Birth again. Type the Capcha Code here the blank box. Finally Click the button of "যাচাই করুন".
Step: 8
Type the OTP Code here which is send in your given Mobile Number. Then Click the button of "ভ্যাকসিন কার্ড ডাউনলোড". Then you can see your Vaccine Card.
Step:9
Finally you can get the PDF file of Vaccine Card with pressing the button of "ভ্যাকসিন কার্ড ডাউনলোড". Now print this Vaccine Card & Preserve it for take vaccine.
What is do for after that?
Answer: After complete your successful submission, please wait for SMS Confirmation of date of vaccination. After finding SMS, go to your selected center for taking vaccine.
N.B: There 2 times of doses. 1st & 2nd doses interval is about 2 Months.
